About Ferritin Test

Ferritin is a protein that stores iron inside your cells, and measuring its level in the blood is currently the single most sensitive and reliable way to assess your body's total iron reserves.

Unlike serum iron, which fluctuates significantly with diet and time of day, ferritin levels remain relatively stable and reflect the amount of iron actually stored in tissues like the liver, spleen, and bone marrow.

A low ferritin level is one of the earliest indicators of developing iron deficiency, often appearing before hemoglobin itself drops low enough to be classified as anemia, which makes ferritin a valuable early-warning test.

On the other end of the spectrum, ferritin also rises during inflammation and certain iron overload conditions, so it needs to be interpreted carefully alongside other clinical information.

When Should You Get Ferritin Test

Ferritin testing is ordered to evaluate:

  • Suspected iron-deficiency anemia, even before other blood counts become abnormal
  • Unexplained fatigue or hair loss
  • Monitoring iron therapy effectiveness
  • Suspected iron overload conditions
  • Chronic inflammatory or infectious conditions, where ferritin can rise as part of the inflammatory response

Conditions Ferritin Test Can Detect

Ferritin levels help diagnose:

  • Iron-deficiency anemia (low ferritin)
  • Hemochromatosis and other iron overload conditions (high ferritin)
  • Inflammatory conditions, since ferritin behaves as an acute-phase reactant and can rise during inflammation independent of iron status

How to Prepare for Ferritin Test

  1. No fasting is typically required, though your doctor may request it if the test is combined with other panels.
  2. A single blood sample is sufficient.
  3. Mention any recent infections or inflammatory conditions to your doctor, since these can temporarily elevate ferritin regardless of your actual iron status.

Ferritin Test Normal Reference Ranges

ParameterNormal Range
Men24–336 ng/mL
Women11–307 ng/mL

Because ferritin can rise with inflammation even when iron stores are genuinely low, your doctor may also check CRP alongside ferritin when the clinical picture is unclear.

Frequently Asked Questions About Ferritin Test

Can ferritin be falsely high?

Yes, ferritin rises during inflammation, infection, and certain liver conditions, independent of your actual iron status, which is why your doctor considers the full clinical picture rather than ferritin alone.

Is ferritin more accurate than a standard iron test?

For assessing your body's overall iron stores, yes — ferritin is generally considered a more reliable marker than serum iron alone, particularly for detecting early iron deficiency.

What ferritin level indicates iron deficiency?

This varies by lab and clinical context, but very low ferritin levels (often below 15–30 ng/mL depending on the reference used) are considered strongly suggestive of iron deficiency.
