About Total IgE Test in Block E, Vasant Vihar1, Delhi

Immunoglobulin E (IgE) is the antibody that drives allergic reactions. It attaches to mast cells and, when it meets an allergen such as pollen, dust mite or a food protein, triggers the release of histamine, causing sneezing, hives, wheezing or swelling. A total IgE test measures the overall amount of IgE in the blood, regardless of what it reacts to.

A high total IgE suggests an allergic (atopic) tendency — such as allergic rhinitis, asthma, eczema or food allergy — or a parasitic infection, but it does not say what you are allergic to. Many people with allergies have normal levels, and some without allergies have high ones, so the test is a clue, not a diagnosis. To identify the trigger, doctors use specific IgE tests or skin-prick testing.

In India, total IgE is often requested along with an absolute eosinophil count for chronic cough, wheeze, skin rashes and suspected worm infections. Very high levels are seen in allergic bronchopulmonary aspergillosis (ABPA), which is a particular concern in people with asthma.

IgE and the allergic march

Children with eczema or food allergy often later develop allergic rhinitis or asthma, a pattern called the allergic march. Total IgE tends to rise through childhood and fall in later adulthood. A strong family history of allergy raises the chance of high IgE.

Next steps to find the trigger

Skin-prick testing or specific IgE blood panels identify the allergen, such as dust mite, pollen, mould, animal dander, or foods like milk, egg, peanut and wheat. Allergy test results must be interpreted with history, since a positive test does not always mean a clinical allergy.

Managing allergy

Avoid triggers where possible (dust-proof bedding, cleaning, humidity control), use medicines such as antihistamines and nasal steroids as advised, and consider immunotherapy for selected cases. Seek emergency care for breathing difficulty, swelling of lips or throat, or fainting after a food or sting.
