Tracheostomy is performed when a patient needs a secure airway for prolonged mechanical ventilation, has an upper airway obstruction from tumors, severe swelling, or trauma, or has conditions causing chronic difficulty clearing secretions, such as certain neuromuscular diseases, that make normal breathing through the mouth or nose unsafe.

Signs You May Need Tracheostomy

Warning signs prompting this procedure include severe difficulty breathing or stridor (a high-pitched breathing sound) from airway blockage, prolonged dependence on a breathing tube through the mouth that can't be safely removed, and an inability to clear secretions leading to repeated aspiration or pneumonia.

Types of Tracheostomy

It can be performed as a surgical tracheostomy done in an operating room with direct visualization, or a percutaneous dilational tracheostomy, a less invasive bedside technique commonly used in ICU patients using a needle and dilators to create the airway opening.

Frequently Asked Questions

Is a tracheostomy permanent?

Not necessarily — many tracheostomies are temporary and can be removed once the underlying problem, like prolonged ventilation or airway swelling, resolves; some conditions do require it long-term or permanently.

Will I be able to speak with a tracheostomy?

Speaking is possible with certain valves or techniques, like a speaking valve, once the patient is stable enough, though it may take some adjustment and practice.

How is the tracheostomy tube cared for?

Regular cleaning and suctioning of the tube and surrounding skin is needed to prevent infection and blockage, and caregivers are usually trained in this before discharge.

What are the risks?

Risks include bleeding, infection, damage to nearby structures, and, rarely, narrowing of the trachea (tracheal stenosis) after the tube is eventually removed.

How long does the procedure take?

The procedure itself is relatively quick, usually 30-60 minutes, though the overall hospital stay depends on the underlying condition requiring the airway.
