Discectomy is recommended when a herniated or bulging spinal disc presses on a nerve root or the spinal cord, causing pain, numbness, or weakness that hasn't improved with rest, physical therapy, or medication over several weeks. It's also indicated urgently for cauda equina syndrome, a rare but serious condition where disc material compresses nerves controlling bladder and bowel function.

Signs You May Need Discectomy

Classic warning signs include sciatica-like pain radiating down the leg (or arm, for a cervical disc), numbness or tingling in the affected limb, and muscle weakness that makes it hard to lift the foot or grip objects. Loss of bladder or bowel control alongside back pain is a medical emergency requiring immediate evaluation.

Types of Discectomy

Open discectomy removes the herniated portion of disc through a small incision with direct visualization, while microdiscectomy uses a microscope or magnifying loupes through an even smaller incision for less tissue disruption and faster recovery. Endoscopic discectomy, the least invasive option, removes disc material through a tube using a tiny camera, and is increasingly used for straightforward herniations in appropriately selected patients.

Frequently Asked Questions

How successful is discectomy for sciatica?

Discectomy has a high success rate, around 90%, for relieving leg pain caused by nerve compression, though results for back pain alone can be less predictable.

How long is recovery after discectomy?

Most patients go home within a day or two and can resume light activities within a few weeks, with full recovery and return to more strenuous work taking four to six weeks.

Will I need spinal fusion along with discectomy?

Not usually — most discectomies are standalone procedures, but fusion may be added if there's spinal instability, which your spine surgeon will assess through imaging beforehand.

Can a herniated disc come back after discectomy?

There is a small risk of recurrent herniation at the same level, particularly with heavy lifting or poor posture during recovery, so following your surgeon's activity restrictions is important.

Are there non-surgical options before discectomy?

Yes, physical therapy, anti-inflammatory medications, and epidural steroid injections are typically tried first, and surgery is generally reserved for cases that don't improve after six or more weeks of conservative treatment or when there are severe neurological symptoms.
