Why Pulmonary Embolism Surgery Is Needed

Surgical pulmonary embolectomy is performed for a massive pulmonary embolism, a large blood clot blocking the pulmonary arteries that severely strains the heart and lungs, when the patient is too unstable for or has not responded to clot-dissolving medication. It is generally reserved for life-threatening cases where the clot burden is too large or risky to treat any other way.

Signs You May Need Pulmonary Embolism Surgery

Warning signs of a large pulmonary embolism include sudden, severe shortness of breath, sharp chest pain that worsens with breathing, rapid heart rate, coughing up blood, and in critical cases, fainting or signs of shock like very low blood pressure. These symptoms typically develop suddenly and require emergency medical attention.

Types of Pulmonary Embolism Surgery

Open surgical embolectomy removes the clot directly through an incision in the chest, often using cardiopulmonary bypass to support the heart and lungs during the procedure, and is reserved for the most severe, life-threatening cases. Catheter-based (percutaneous) clot removal or clot-dissolving therapy delivered directly to the clot is a less invasive alternative used more frequently, chosen based on the patient's stability and clot location.

Frequently Asked Questions

Is surgery the first treatment for pulmonary embolism?

No, most pulmonary embolisms are treated with blood-thinning medications, and surgery or catheter-based clot removal is reserved for the most severe, life-threatening cases where clot burden is very large or the patient is too unstable for medication alone.

How urgent is this surgery when needed?

It is a true emergency procedure, often performed within hours of diagnosis, since a massive pulmonary embolism can rapidly become fatal without prompt treatment to restore blood flow to the lungs.

What is recovery like after pulmonary embolectomy?

Recovery involves a hospital stay of one to two weeks or more depending on how critically ill the patient was, followed by four to six weeks of gradual recovery and cardiac or pulmonary rehabilitation.

Will I need blood thinners long-term after this surgery?

Yes, most patients require blood-thinning medication for an extended period, sometimes lifelong, to prevent new clots from forming, which your doctor will tailor based on your underlying risk factors.

What causes a pulmonary embolism in the first place?

Most pulmonary emboli originate as blood clots in the deep veins of the legs that travel to the lungs, often triggered by prolonged immobility, surgery, cancer, or clotting disorders, which your doctor can help identify and manage.
