Ileal resection removes a diseased portion of the ileum (the last part of the small intestine), most commonly for Crohn's disease causing strictures or fistulas, cancer, or a bowel obstruction or perforation that can't be resolved without surgery. It's typically performed once medication and other treatments fail to control the underlying disease or when a complication requires urgent surgical correction.

Signs You May Need Ileal Resection

Warning signs include persistent abdominal pain, especially in the lower right abdomen, chronic diarrhea, weight loss, and symptoms of bowel obstruction like bloating, nausea, and vomiting. Blood in the stool or signs of a fistula (abnormal connection to skin or another organ) causing unusual drainage are also indicators for surgical evaluation.

Types of Ileal Resection

Segmental resection removes only the diseased section of ileum with reconnection (anastomosis) of the healthy remaining bowel ends, and is the standard approach when disease is localized. Laparoscopic ileal resection uses small incisions and a camera for a less invasive recovery in appropriately selected patients, while open resection through a larger abdominal incision is used for more extensive disease, significant scarring from previous surgery, or emergency situations.

Frequently Asked Questions

Will I need a stoma after ileal resection?

Most planned resections allow the bowel to be reconnected directly without a stoma, but a temporary ileostomy may be needed in complicated, infected, or emergency cases to let the bowel heal safely.

Can Crohn's disease come back after ileal resection?

Yes, Crohn's disease can recur at or near the surgical site over time, which is why ongoing medical management and regular follow-up with a gastroenterologist remain important even after successful surgery.

How long is recovery after ileal resection?

Most patients spend five to ten days in hospital, with a return to normal activities in four to six weeks, depending on the extent of surgery and any complications.

Will removing part of my ileum cause nutritional problems?

The ileum absorbs vitamin B12 and bile salts, so removing a significant portion can sometimes lead to deficiencies, particularly of B12, which is why periodic monitoring and supplementation may be recommended.

What are the risks of ileal resection?

Risks include infection, bleeding, and leakage at the site where the bowel is reconnected, which your surgical team monitors closely in the days after surgery; discussing your specific risk factors with your surgeon beforehand is worthwhile.
