Why Glaucoma Surgery (Trabeculectomy) Is Needed

Trabeculectomy is specifically recommended when glaucoma continues to progress and eye pressure remains too high despite maximum medical therapy (eye drops) and laser treatments. It's the go-to filtration surgery for patients at high risk of ongoing optic nerve damage who need a more substantial and lasting pressure reduction than drops alone can provide.

Signs You May Need Glaucoma Surgery (Trabeculectomy)

Because glaucoma is often symptomless in its early stages, the main indicator for trabeculectomy is a documented, progressive rise in eye pressure or worsening visual field loss detected during routine ophthalmic monitoring, despite being on multiple medications. Patients who struggle with consistent use of eye drops, or who experience side effects from long-term drop use, are also considered strong candidates.

Types of Glaucoma Surgery (Trabeculectomy)

Standard trabeculectomy creates a small flap in the eye's outer wall (sclera) to let fluid drain into a controlled space under the conjunctiva, forming a "bleb" that regulates pressure. Modern refinements include using anti-scarring agents like mitomycin-C during surgery to keep the new drainage channel from closing, and adjustable or releasable sutures that let the surgeon fine-tune drainage in the days after surgery.

Frequently Asked Questions

What is a "bleb" after trabeculectomy?

It's a small, often barely visible blister-like area under the upper eyelid where fluid drains and is absorbed, and your ophthalmologist will monitor it at follow-up visits to ensure it's functioning properly.

How long does it take to recover from trabeculectomy?

Most people notice improvement over four to six weeks, with frequent follow-up visits early on since the pressure and healing of the drainage site need close monitoring during this period.

Can trabeculectomy fail over time?

Some blebs can scar down and become less effective over months to years, in which case additional treatment, medication, or a repeat surgery may be needed to control pressure again.

Will I need eye drops forever after trabeculectomy?

Many patients can reduce or stop glaucoma drops after a successful trabeculectomy, though some may still need medication if pressure isn't fully controlled by surgery alone.

Is trabeculectomy risky?

It's a well-established, effective procedure with an 85% success rate, though as with any eye surgery there are risks like infection, bleeding, or under/over-drainage, which your ophthalmologist will discuss and monitor closely afterward.
