Why Eye Muscle Surgery Is Needed

This surgery is performed to correct strabismus (misaligned eyes) caused by imbalance in the muscles controlling eye movement, which can be present from birth or develop later due to nerve problems, thyroid eye disease, or injury. It's recommended when the misalignment causes double vision, poor depth perception, or, in children, risks permanent reduction in vision (amblyopia) in the weaker eye.

Signs You May Need Eye Muscle Surgery

Visible signs include eyes that don't point in the same direction, noticeable eye drifting (inward, outward, up, or down), double vision, and a child tilting or turning their head to compensate for misalignment. Difficulty with depth perception and eye strain or fatigue from trying to keep the eyes aligned are also common complaints.

Types of Eye Muscle Surgery

Recession surgery weakens an overactive eye muscle by detaching and reattaching it further back on the eye, while resection strengthens a weak muscle by shortening it, and surgeons often combine both approaches on different muscles to achieve the best alignment. Adjustable suture techniques allow fine-tuning of the muscle position shortly after surgery for more precise results, particularly useful in adults or complex cases.

Frequently Asked Questions

At what age can eye muscle surgery be done?

It can be performed at almost any age, including in infants when strabismus is diagnosed early, since prompt correction in children helps prevent amblyopia and supports normal visual development.

Will I still need glasses after eye muscle surgery?

Possibly — the surgery corrects the alignment of the eyes but doesn't change refractive errors like near- or far-sightedness, so glasses may still be needed for clear vision.

How successful is strabismus surgery?

Success rates are good, around 85%, though some patients need a second, fine-tuning procedure to achieve optimal alignment, especially in more complex or long-standing cases.

Is eye muscle surgery painful?

It's performed under anesthesia so there's no pain during surgery, and while the eye may feel scratchy, red, or mildly uncomfortable for a few days afterward, it's generally well tolerated.

How long is recovery after eye muscle surgery?

Most people return to normal activities within one to two weeks, though full settling of eye alignment and comfort can take a few weeks longer; your ophthalmologist will schedule follow-up to confirm the result.
