Why Pilonidal Cyst Surgery Is Needed

Pilonidal cysts form at the base of the spine, near the tailbone, often when loose hairs get pushed into the skin and trigger inflammation, infection, and cyst or abscess formation, particularly in people who sit for long periods or have thick body hair. Surgery is recommended once the cyst becomes infected, painful, or keeps recurring despite drainage or antibiotics.

Signs You May Need Pilonidal Cyst Surgery

Patients typically notice a painful, swollen lump at the top of the buttocks crease, often with redness, warmth, and sometimes drainage of pus or blood, along with a foul odor. Fever and worsening pain when sitting or moving are signs the cyst has become an abscess requiring prompt drainage and treatment.

Types of Pilonidal Cyst Surgery

Incision and drainage quickly relieves an acute abscess but often does not prevent recurrence since the underlying sinus tract remains. Wide excision surgically removes the entire cyst and sinus tract for a more definitive cure, and flap procedures reconstruct the area with nearby tissue to reduce recurrence risk in patients with chronic or recurrent pilonidal disease.

Frequently Asked Questions

Will a pilonidal cyst come back after surgery?

Simple drainage has a higher chance of recurrence since the sinus tract remains, while complete excision of the cyst and tract offers a more lasting solution, which your surgeon can recommend based on your history.

How painful is recovery after pilonidal cyst surgery?

There is noticeable discomfort for the first week, especially with sitting, but this improves steadily, and most patients manage well with prescribed pain medication and wound care.

How long does the wound take to heal?

Depending on the surgical technique, healing can take anywhere from a few weeks for smaller excisions to several weeks for wounds left open to heal from the inside out.

Can I prevent pilonidal cysts from recurring?

Keeping the area clean, regular hair removal around the site, and avoiding prolonged sitting on hard surfaces can help reduce the chance of recurrence after treatment.

Is surgery always necessary for a pilonidal cyst?

Small, non-infected cysts may sometimes be managed conservatively, but recurrent or infected cysts generally require surgical treatment for lasting relief, as your surgeon will advise based on the severity.
