Why Parathyroid Surgery Is Needed

Parathyroid surgery is performed to treat hyperparathyroidism, where one or more of the small parathyroid glands become overactive, usually due to a benign adenoma, and release excess parathyroid hormone that raises blood calcium to unhealthy levels. Less commonly it is done for parathyroid cancer or gland enlargement caused by chronic kidney disease.

Signs You May Need Parathyroid Surgery

Elevated calcium from overactive parathyroid glands can cause fatigue, bone pain, kidney stones, excessive thirst and urination, constipation, and mood changes like depression or difficulty concentrating. Many cases are actually detected incidentally through routine blood tests showing high calcium before more severe symptoms develop.

Types of Parathyroid Surgery

Minimally invasive parathyroidectomy targets and removes only the single overactive gland identified through pre-operative imaging and scanning, using a small neck incision. Bilateral neck exploration examines all four parathyroid glands directly when imaging cannot pinpoint the problem gland or multiple glands are suspected to be involved, guided during surgery by rapid hormone-level testing to confirm success.

Frequently Asked Questions

How is the overactive gland found before surgery?

Imaging tests like a sestamibi scan or ultrasound are used beforehand to localize the abnormal gland, allowing many patients to have a smaller, more targeted surgery rather than exploring the whole neck.

How quickly will my calcium levels normalize after surgery?

Calcium levels typically drop to normal within a day or two of successful surgery, and doctors monitor them closely afterward to confirm the overactive tissue was completely removed.

Is parathyroid surgery a major operation?

It is generally a relatively short, low-risk procedure, especially the minimally invasive version, with most patients going home the same or next day and returning to normal activity within one to two weeks.

What happens if all four parathyroid glands are removed?

Since some parathyroid tissue is needed to regulate calcium, surgeons aim to preserve at least a portion of healthy gland tissue, and in rare cases where all tissue must be removed, small pieces may be reimplanted elsewhere.

Can hyperparathyroidism come back after surgery?

Recurrence is uncommon after successful removal of the overactive gland, though periodic calcium level checks are recommended to catch any recurrence or new gland involvement early.
