Why Deep Brain Stimulation (DBS) Is Needed

Neurologists recommend DBS for patients with Parkinson's disease, essential tremor, or dystonia whose involuntary movements and rigidity no longer respond adequately to optimized medical therapy. It is also considered when medication side effects, such as unpredictable symptom swings or dyskinesia, become as disruptive as the underlying disease itself.

Signs You May Need Deep Brain Stimulation (DBS)

Patients are usually referred when tremor, stiffness, or abnormal postures/movements interfere with basic tasks like eating, dressing, or walking despite trying multiple medication combinations. Sudden, unpredictable "off" periods where medication effect wears off, or involuntary jerking movements from long-term drug therapy, are key indicators that a neurologist may raise DBS as an option.

Types of Deep Brain Stimulation (DBS)

DBS involves implanting electrodes into precise brain targets — chosen based on whether the primary condition is Parkinson's, tremor, or dystonia — connected to an implantable pulse generator placed under the chest skin. Procedures may use an awake, local-anesthesia technique for real-time symptom testing, or newer asleep, image-guided techniques under general anesthesia that rely on intraoperative imaging instead of patient feedback.

Frequently Asked Questions

How is DBS different for Parkinson's versus essential tremor or dystonia?

The surgical technique is similar, but the specific brain target and stimulation settings are tailored to the condition being treated, which your neurology and neurosurgery team decide together after imaging and clinical assessment.

Does DBS stop the need for medication?

Most patients still need some medication after DBS, but many can reduce their dosage substantially, which also lessens medication side effects — your neurologist will adjust your regimen gradually after surgery.

How long do DBS batteries last?

Non-rechargeable pulse generators typically last three to five years before replacement, while rechargeable versions can last significantly longer; your care team will monitor battery status at follow-up visits.

What is the recovery time after DBS?

Most patients are hospitalized for a few days and need four to six weeks before resuming normal activities, with stimulation programming sessions continuing over the following months to optimize benefit.

Can DBS be reversed or adjusted?

Yes, unlike older lesioning surgeries, DBS is adjustable and can be turned off or reprogrammed non-invasively, and the hardware can be removed if necessary, though this decision should be made with your neurologist.
