Why Pilonidal Sinus Surgery Is Needed

Pilonidal sinus disease develops when ingrown hairs and skin debris create a tunnel-like tract under the skin at the base of the tailbone, leading to recurrent infection, abscess formation, and chronic drainage. Surgery becomes necessary when the sinus repeatedly flares up, fails to heal, or causes ongoing discomfort despite conservative wound care.

Signs You May Need Pilonidal Sinus Surgery

Common symptoms include a persistent small opening or dimple at the top of the buttock crease that drains fluid, pus, or blood, along with pain, swelling, and irritation, especially during prolonged sitting. Recurrent episodes of painful swelling and discharge, sometimes with fever, indicate active infection needing surgical management.

Types of Pilonidal Sinus Surgery

Sinusotomy or laying open the tract removes the roof of the sinus so it can heal from the base upward, a straightforward option for uncomplicated cases. Excision with primary closure removes the entire sinus tract and stitches the wound closed for faster healing, while flap reconstruction techniques like the Karydakis or Limberg flap reposition healthy tissue over the area to reduce the risk of recurrence in chronic or complex cases.

Frequently Asked Questions

What is the difference between a pilonidal cyst and a pilonidal sinus?

They represent different stages of the same underlying condition, with a cyst being a closed pocket of infection and a sinus being a chronic draining tract, and treatment approaches often overlap depending on severity.

How long does it take to fully heal after sinus surgery?

Healing time varies by technique, from a few weeks for closed procedures to six or more weeks for wounds left open to heal gradually, with your surgeon guiding wound care throughout.

Can pilonidal sinus surgery be done under local anesthesia?

Smaller, less complex procedures can sometimes be done under local or spinal anesthesia, while more extensive flap reconstructions may require general or spinal anesthesia depending on your surgeon's assessment.

Will I need to avoid sitting after surgery?

Prolonged sitting is generally discouraged in the initial recovery period to reduce pressure on the healing wound, with your surgeon giving specific guidance based on your procedure.

How can I reduce the risk of pilonidal disease returning?

Maintaining good hygiene, regular hair removal in the area, and avoiding prolonged pressure on the tailbone region are key steps recommended after treatment to lower recurrence risk.
