Fundoplication is recommended for chronic gastroesophageal reflux disease (GERD) that doesn't respond adequately to medications, or for a large hiatal hernia that allows stomach acid to reflux into the esophagus. It's also used for patients who don't want to remain on long-term acid-suppressing medication or who have complications from reflux such as esophagitis or Barrett's esophagus.

Signs You May Need Fundoplication

Persistent heartburn, regurgitation of acid or food into the throat, chronic cough, hoarseness, and a sensation of a lump in the throat despite medication are the main indicators. Chest pain, difficulty swallowing, and nighttime reflux disrupting sleep are also common reasons doctors consider surgical treatment.

Types of Fundoplication

Nissen fundoplication, the most common technique, wraps the top of the stomach 360 degrees around the lower esophagus to reinforce the valve that prevents reflux, typically performed laparoscopically. Partial wraps, such as Toupet (270 degrees) or Dor (anterior, 180-200 degrees) fundoplication, are used in patients with weaker esophageal motility to reduce the risk of difficulty swallowing after surgery.

Frequently Asked Questions

Will fundoplication cure my GERD completely?

Most patients experience substantial or complete relief from reflux symptoms, with around a 90% success rate, allowing many to stop or significantly reduce acid-suppressing medications.

What are the side effects of fundoplication?

Some patients experience temporary difficulty swallowing, bloating, or inability to belch or vomit normally after surgery, which usually improves over weeks to months as swelling settles.

How long is recovery after fundoplication?

Hospital stay is typically two to three days, with most patients returning to normal activities within three to four weeks, following a gradual diet progression from liquids to solids.

Is fundoplication reversible?

While technically the wrap can be revised or taken down in a further surgery if problems arise, it's generally intended as a long-term or permanent solution, so this decision should be made after thorough discussion with a gastrointestinal surgeon.

Are there alternatives to fundoplication for GERD?

Lifestyle changes, medications, and newer procedures like the LINX magnetic device are alternatives depending on your specific condition and severity; a gastroenterologist can help determine if you're a better fit for surgery or a less invasive option.
