Why Cochlear Implant Surgery Is Needed

Performed for patients with severe to profound sensorineural hearing loss who don't gain adequate benefit from hearing aids, most often due to damaged hair cells in the cochlea from genetic factors, infections, ototoxic medications, or age-related hearing loss. The implant bypasses the damaged hair cells and directly stimulates the auditory nerve to restore functional hearing.

Signs You May Need Cochlear Implant Surgery

Poor speech understanding despite well-fitted hearing aids, requiring maximum hearing aid amplification with little benefit, and in infants, no response to loud sounds or failing formal hearing tests are the key indications. Progressive hearing loss that hearing aids can no longer adequately manage in older children or adults also prompts evaluation for implantation.

Types of Cochlear Implant Surgery

Surgery can be unilateral or bilateral, with the internal device placed under the skin behind the ear and connected to an external speech processor worn after healing. Advances in electrode array design aim to preserve any residual natural hearing where possible, particularly relevant for candidates for combined electric-acoustic stimulation.

Frequently Asked Questions

How long does the surgery take?

The procedure itself typically takes about 2-4 hours, performed under general anesthesia.

When does the implant get turned on?

The external processor is usually fitted and activated about 2-4 weeks after surgery, once the surgical site has healed.

Is there a risk to residual hearing during surgery?

Yes, there's some risk to any remaining natural hearing in the implanted ear, which surgeons try to minimize with careful, gentle surgical technique, especially when hearing preservation is a goal.

How long is auditory rehabilitation after implantation?

This varies widely, from months to a couple of years, depending on the patient's age at implantation and how long they've had hearing loss, with consistent therapy improving outcomes.

Can both ears be implanted?

Yes, bilateral implantation is increasingly common, particularly in young children, to improve sound localization and hearing in background noise.
