Radical cystectomy is recommended chiefly for muscle-invasive bladder cancer or high-grade non-muscle-invasive tumors that keep recurring despite BCG therapy or TURBT. It is also used for extensive bladder tumors that cannot be controlled locally, certain congenital bladder abnormalities, or a severely damaged, non-functioning bladder from radiation, chronic infection, or interstitial cystitis.

Signs You May Need Cystectomy

Warning signs include visible blood in the urine (hematuria) that persists or recurs, frequent or urgent urination, pelvic pain, and recurrent urinary tract infections that don't respond to treatment. Unexplained weight loss and pain in the flank or lower back can indicate advancing bladder cancer that has spread beyond the bladder lining.

Types of Cystectomy

Radical cystectomy removes the entire bladder along with nearby lymph nodes and, in men, the prostate and seminal vesicles, or in women, the uterus and part of the vagina, and can be performed as open surgery, laparoscopically, or with robotic assistance. Partial cystectomy removes only the cancerous portion in carefully selected cases. After bladder removal, surgeons create a urinary diversion — an ileal conduit, a continent cutaneous pouch, or a neobladder constructed from bowel tissue that allows near-normal voiding.

Frequently Asked Questions

How much does bladder removal surgery (cystectomy) cost in India?

Cystectomy with urinary diversion typically costs more than many surgeries because of the complexity and extended hospital stay, and the exact price depends on the diversion type, hospital, and city; a urologist or hospital counselor can give you a personalized estimate after evaluation.

Will I need a permanent bag after cystectomy?

Not necessarily — an ileal conduit does require an external collection bag, but a neobladder reconstructed from your own intestine allows many patients to urinate through the urethra again, and your surgeon will discuss which option best suits your health and anatomy.

How long is recovery after cystectomy?

Most patients spend one to two weeks in hospital and need six to eight weeks before returning to normal activity, with full adjustment to the urinary diversion taking a bit longer.

What are the risks of cystectomy?

As major surgery, risks include infection, bleeding, blood clots, and complications related to the urinary diversion such as leakage or stricture; discussing your specific risk profile with your surgical team beforehand is important.

Are there alternatives to removing the whole bladder?

For early, non-muscle-invasive cancers, TURBT with intravesical therapy (like BCG) may control the disease without full bladder removal, though close surveillance is essential and a specialist can advise if you're a candidate.
