Why Glaucoma Surgery Is Needed

Glaucoma surgery is recommended when elevated eye pressure continues to threaten the optic nerve despite maximum tolerated eye drops and laser treatment, risking progressive, irreversible vision loss. It's also considered for certain types of glaucoma, like angle-closure glaucoma, where the drainage angle is structurally blocked and needs surgical correction rather than just pressure-lowering drops.

Signs You May Need Glaucoma Surgery

Glaucoma often has no early symptoms, which is why regular eye pressure checks are important, but warning signs of advancing disease include gradual loss of peripheral (side) vision, tunnel vision in later stages, and, in acute angle-closure glaucoma, sudden severe eye pain, blurred vision, halos around lights, nausea, and vomiting. Any sudden eye pain with vision changes needs emergency evaluation.

Types of Glaucoma Surgery

Trabeculectomy creates a new drainage channel for fluid to escape the eye and is a long-standing, effective option for pressure-lowering. Tube shunt surgery implants a small drainage device, often used for more complex or previously operated eyes, while minimally invasive glaucoma surgery (MIGS) uses tiny devices or techniques with quicker recovery, suited to earlier or milder glaucoma, sometimes combined with cataract surgery.

Frequently Asked Questions

Does glaucoma surgery restore lost vision?

No, surgery cannot reverse vision already lost to glaucoma, but it can effectively lower eye pressure to help preserve remaining vision and slow further damage.

Will I still need eye drops after glaucoma surgery?

Some patients can reduce or stop drops after successful surgery, while others still need medication to fully control pressure; your ophthalmologist will monitor and adjust your treatment plan afterward.

How successful is glaucoma surgery?

Success rates are good, around 85%, for controlling eye pressure, though results can vary by the specific type of glaucoma and surgical technique used.

Is glaucoma surgery painful?

It's performed under local anesthesia so there's minimal discomfort during the procedure, and any post-operative irritation is typically manageable with prescribed drops and mild pain relief.

How urgent is glaucoma surgery?

Timing depends on the type and severity of glaucoma — angle-closure glaucoma with sudden pressure spikes needs urgent treatment, while chronic open-angle glaucoma allows more time to try medications first; your ophthalmologist will guide the appropriate timeline for you.
