Why Radical Cystectomy Is Needed

Radical cystectomy is performed for invasive or recurrent bladder cancer that has grown into the bladder's muscle wall or keeps returning despite less invasive treatments like resection and intravesical therapy. It is recommended when removing the entire bladder offers the best chance of controlling cancer that is too extensive for bladder-preserving treatments.

Signs You May Need Radical Cystectomy

Warning signs of bladder cancer include blood in the urine (often painless), frequent or urgent urination, pain during urination, and pelvic pain, particularly as the cancer becomes more invasive. Persistent or recurrent symptoms despite treatment for suspected infection should prompt further urologic evaluation to rule out invasive cancer.

Types of Radical Cystectomy

Open radical cystectomy removes the bladder and surrounding organs through a lower abdominal incision, while robotic-assisted and laparoscopic radical cystectomy achieve the same removal through smaller incisions with potentially less blood loss and faster recovery. Following bladder removal, urinary diversion is created using either an ileal conduit (external bag), a continent reservoir, or a neobladder constructed from intestine to allow more normal urination.

Frequently Asked Questions

Will I need a permanent urine bag after this surgery?

This depends on the type of urinary diversion chosen; an ileal conduit uses an external bag, while a neobladder reconstruction can allow urination through the urethra without an external appliance, and your urologist will discuss which is suitable for you.

How is the type of urinary diversion decided?

Factors like the extent of cancer, your overall health, kidney function, and personal preference all factor into whether an ileal conduit, continent reservoir, or neobladder is the best option for you.

How long is recovery after radical cystectomy?

This is a major surgery with a hospital stay of about seven to ten days and full recovery typically taking six to ten weeks, given the complexity of bladder removal and urinary reconstruction.

Will this surgery affect sexual function?

Because nearby organs and nerves involved in sexual function may be affected, especially in men where the prostate is also removed, this is an important discussion to have with your urologic oncologist before surgery.

Will I need chemotherapy along with surgery?

Many patients with invasive bladder cancer receive chemotherapy before surgery to shrink the tumor and improve outcomes, and your oncology team will determine the best treatment sequence for your specific case.
