Why Lumbar Disc Surgery Is Needed

This surgery is recommended when a herniated or bulging disc in the lower back presses on a spinal nerve root, most often due to age-related disc degeneration, repetitive strain, heavy lifting, or a sudden injury. It becomes necessary when the compressed nerve causes significant pain or weakness that has not improved with rest, physiotherapy, or medication over several weeks.

Signs You May Need Lumbar Disc Surgery

The classic symptom is sciatica, sharp shooting pain radiating from the lower back down one leg, often accompanied by numbness, tingling, or muscle weakness in the leg or foot. Severe cases with loss of bladder or bowel control or progressive leg weakness indicate cauda equina syndrome, a surgical emergency requiring immediate attention.

Types of Lumbar Disc Surgery

Microdiscectomy, the most common technique, removes only the herniated portion of the disc through a small incision using a microscope for minimal tissue disruption. Open discectomy involves a larger incision for more extensive disc removal, while endoscopic discectomy uses an even smaller incision with a tiny camera, and spinal fusion may be added if the disc segment is unstable.

Frequently Asked Questions

Is surgery always needed for a herniated disc?

No, most herniated discs improve with conservative treatment like physiotherapy, medications, and rest over six to twelve weeks, with surgery reserved for persistent pain, worsening weakness, or nerve emergencies.

How successful is lumbar disc surgery for sciatica relief?

Most patients experience significant relief from leg pain, with success rates around 90%, though back pain itself may improve more gradually and physiotherapy afterward supports full recovery.

How soon can I return to work after disc surgery?

Desk-based work can often resume within two to four weeks depending on the technique used, while physically demanding jobs may require six weeks or more of recovery.

Can the disc herniate again after surgery?

There is a small chance of recurrent herniation at the same level, which is why maintaining good posture, core strength, and avoiding heavy strain during recovery is important.

What are the alternatives if I want to avoid surgery?

Epidural steroid injections, targeted physiotherapy, and pain management are common alternatives tried first, and a spine specialist can help you decide the right timing for surgery if these do not help.
