Mastoid surgery is performed to treat chronic or recurrent middle ear infections that have spread into the mastoid bone, a cholesteatoma (abnormal skin growth) eroding surrounding structures, or complications of ear infections such as hearing bone damage or the risk of infection spreading toward the brain. It is recommended when antibiotics and ear drops fail to control persistent infection or when structural damage is found on imaging.

Signs You May Need Mastoid Surgery

Common warning signs include persistent or foul-smelling ear discharge, hearing loss, ear pain, and a feeling of fullness or pressure behind the ear. More serious symptoms such as dizziness, facial weakness, fever, or headache can indicate the infection is spreading and require urgent ENT evaluation.

Types of Mastoid Surgery

Canal wall up mastoidectomy removes diseased mastoid air cells while preserving the natural ear canal wall for a more normal-appearing ear canal afterward. Canal wall down mastoidectomy removes the canal wall as well for more complete disease clearance in extensive cholesteatoma, often followed by reconstructive procedures to restore hearing where possible.

Frequently Asked Questions

Will mastoid surgery restore my hearing?

The primary goal is clearing infection and preventing complications; hearing may improve if reconstruction of the middle ear bones is done, but this depends on the extent of damage found during surgery.

How long is the recovery after mastoid surgery?

Most patients stay in the hospital for two to four days with full recovery taking four to six weeks, during which water must be kept out of the ear as it heals.

What happens if a cholesteatoma is left untreated?

Untreated cholesteatoma can continue eroding bone, leading to permanent hearing loss, facial nerve damage, or rare but serious complications like meningitis, which is why surgical removal is usually recommended once diagnosed.

Will I need follow-up surgeries?

Some patients, especially those with canal wall down procedures or extensive cholesteatoma, may need a planned second-look surgery or staged hearing reconstruction, which your ENT surgeon will discuss based on your case.

Is mastoid surgery safe?

It is a well-established ENT procedure with good success rates in controlling infection, though like all ear surgery it carries small risks to hearing, balance, and facial nerve function that your surgeon will explain.
