Why Deep Brain Stimulation Is Needed

DBS is considered for movement disorders — most commonly advanced Parkinson's disease, essential tremor, and dystonia — when medications no longer control symptoms adequately or cause disabling side effects like dyskinesia. It's recommended after a thorough neurological evaluation confirms the patient's symptoms are of the type known to respond well to targeted electrical stimulation of specific brain structures.

Signs You May Need Deep Brain Stimulation

Candidates typically have worsening tremor, muscle rigidity, slowness of movement, or unpredictable "on-off" fluctuations in symptom control despite optimized medication. Disabling involuntary movements (dyskinesias) from long-term levodopa use, or tremor severe enough to interfere with eating, writing, or daily tasks, are common triggers for referral to a DBS specialist.

Types of Deep Brain Stimulation

The standard approach uses stereotactic frame-based or frameless neuronavigation to precisely implant thin electrodes into targets such as the subthalamic nucleus or globus pallidus, most often done with the patient partially awake (local anesthesia) so surgeons can test stimulation in real time. A separate procedure then implants a battery-powered pulse generator under the skin near the collarbone, and newer directional and rechargeable devices allow more precise, longer-lasting stimulation than earlier models.

Frequently Asked Questions

Is DBS a cure for Parkinson's disease?

No, DBS doesn't cure Parkinson's or halt its progression, but it can significantly reduce tremor, rigidity, and medication-related fluctuations, often allowing lower drug doses and better quality of life.

Is DBS surgery safe?

DBS is a well-established procedure with success rates around 85% for symptom improvement in appropriately selected patients, though as brain surgery it carries risks like bleeding, infection, or hardware issues that your neurosurgeon will discuss in detail.

Will I be awake during DBS surgery?

Many centers keep patients awake (under local anesthesia) during electrode placement so the team can test stimulation and confirm optimal positioning, while the battery implantation is usually done under light sedation or general anesthesia.

How soon do DBS benefits appear?

Some benefits are apparent soon after the device is activated, typically a few weeks after surgery, but fine-tuning the stimulation settings can take several months to achieve the best results.

Who is not a good candidate for DBS?

Patients with significant cognitive decline, certain psychiatric conditions, or symptoms that don't respond to levodopa are generally poor candidates; a movement disorder specialist performs detailed testing beforehand to confirm suitability.
