Why Epilepsy Surgery Is Needed

Epilepsy surgery is considered when seizures originate from a specific, identifiable area of the brain and continue despite trying at least two or more appropriately chosen anti-seizure medications (drug-resistant epilepsy). It's also indicated when a structural cause — such as a scar, tumor, or malformation — is found on imaging that correlates with seizure activity.

Signs You May Need Epilepsy Surgery

Patients considered for this surgery typically have frequent, disabling seizures that severely limit daily activities like driving or working, despite years of medication trials. Seizures with a consistent pattern or aura pointing to one brain region, or seizures causing dangerous falls or loss of consciousness, are strong indicators for a detailed pre-surgical evaluation.

Types of Epilepsy Surgery

Resective surgery removes the specific brain tissue causing seizures (such as temporal lobectomy, the most common and successful type) after careful mapping to avoid critical functions like speech or movement. Alternative approaches include laser interstitial thermal therapy (a minimally invasive option that destroys the seizure focus with heat through a small probe), and neurostimulation devices like responsive neurostimulation or vagus nerve stimulation for patients who aren't candidates for tissue removal.

Frequently Asked Questions

How successful is epilepsy surgery?

Success varies by seizure type and location, with rates around 70% for significant seizure reduction or freedom, particularly for well-localized temporal lobe epilepsy; a comprehensive pre-surgical workup helps predict your individual chances.

Is epilepsy surgery risky?

As brain surgery, it carries risks including infection, bleeding, and, depending on location, possible effects on memory, speech, or vision, which is why extensive mapping is done beforehand to minimize these risks.

How long does the pre-surgical evaluation take?

This can take weeks to months and includes video-EEG monitoring, MRI, and sometimes additional tests to precisely localize the seizure focus and map critical brain functions before surgery is recommended.

Will I still need medication after epilepsy surgery?

Many patients can reduce their medication over time, but most continue at least some anti-seizure medication for a period after surgery, gradually tapering under close neurologist supervision if seizures remain controlled.

What is the recovery time after epilepsy surgery?

Hospital stay is typically five to ten days, with most patients returning to normal activities within six to eight weeks, though cognitive recovery and medication adjustments continue over a longer period.
