Why Dacryocystorhinostomy Is Needed

DCR is recommended when the nasolacrimal duct — the channel that drains tears from the eye into the nose — becomes blocked, most commonly from chronic inflammation, scarring after infection, injury, or, less often, a tumor pressing on the duct. Recurrent dacryocystitis (infection of the tear sac) that keeps relapsing despite antibiotics is one of the strongest indications for surgery.

Signs You May Need Dacryocystorhinostomy

The hallmark symptom is constant watering of the eye (epiphora) that isn't explained by allergy or dry eye, often accompanied by sticky discharge, crusting at the inner corner of the eye, or a painful swollen lump near the nose bridge from an infected tear sac. Repeated episodes of redness, swelling, or pus discharge over the tear sac are strong indicators.

Types of Dacryocystorhinostomy

External DCR creates a small skin incision near the nose to construct a new drainage passage between the tear sac and nasal cavity, offering excellent long-term success and a well-established track record. Endonasal (endoscopic) DCR achieves the same bypass entirely through the nostril with no visible scar and comparable success rates in experienced hands, and a silicone stent may be placed temporarily in either technique to keep the new channel open while it heals.

Frequently Asked Questions

Is DCR surgery painful?

The procedure is done under general or local anesthesia so you won't feel pain during surgery, and post-operative discomfort is usually mild and manageable with regular painkillers.

Will there be a visible scar after DCR?

Endonasal DCR leaves no external scar since it's done through the nose, while external DCR leaves a small, usually well-hidden scar near the bridge of the nose that fades over time.

How soon will my watery eye improve after DCR?

Most patients notice a significant reduction in tearing within a few weeks as swelling subsides and the new drainage channel starts functioning, though full healing takes about a month.

Can the blocked tear duct come back after DCR?

Recurrence is uncommon with a success rate around 90%, but scarring at the new opening can occasionally cause it to narrow again, in which case a revision procedure or stent may be needed.

Are there non-surgical options for a blocked tear duct?

In infants, blockages often resolve on their own or with massage and probing, but in adults with true nasolacrimal duct obstruction, DCR is generally the definitive and most effective solution — an ophthalmologist can confirm what applies to your case.
