Mediastinoscopy is performed to investigate enlarged lymph nodes or a mass in the mediastinum, the central chest area between the lungs, most often to diagnose or stage lung cancer, lymphoma, or sarcoidosis. It is chosen when imaging like CT or PET scans shows an abnormality that needs a tissue sample for a definitive diagnosis before planning further treatment.

Signs You May Need Mediastinoscopy

Many patients undergoing this procedure have few direct symptoms and are referred based on an abnormal finding on chest imaging done for another reason, such as a cough or cancer workup. When symptoms are present, they may include a persistent cough, chest discomfort, or unexplained swelling of lymph nodes detected during a broader cancer evaluation.

Types of Mediastinoscopy

Cervical mediastinoscopy, the standard technique, involves a small incision at the base of the neck through which a scope reaches the central chest to biopsy lymph nodes. Video-assisted mediastinoscopy uses a camera for a magnified, recorded view, and anterior mediastinotomy (Chamberlain procedure) accesses nodes on the side of the chest not reachable by the standard cervical approach.

Frequently Asked Questions

Is mediastinoscopy a major operation?

It is a minimally invasive diagnostic procedure, typically taking one to two hours with a short hospital stay of a day or less, much less involved than a full chest surgery.

Why can't a needle biopsy be used instead?

In some cases needle biopsy is tried first, but mediastinoscopy provides a larger, more reliable tissue sample directly under vision, which is especially important for accurate lung cancer staging.

What happens if the biopsy shows cancer?

A positive result helps your oncology team determine the exact cancer stage, which guides decisions about whether surgery, chemotherapy, radiation, or a combination is the best treatment path.

How long is recovery after mediastinoscopy?

Recovery is quick, with most patients going home the same or next day and returning to normal activity within one to two weeks, with only mild throat or incision discomfort.

Are there risks with this procedure?

It is generally safe, though rare risks include bleeding, injury to nearby structures like nerves or blood vessels, or infection, which your thoracic surgeon will discuss beforehand.
