Fistula surgery treats abnormal tunnel-like connections that form between the anal canal and skin (anal fistula), usually following a previous anal abscess that didn't heal completely, or between internal organs from conditions like Crohn's disease, diverticulitis, or after certain pelvic surgeries. Persistent drainage or infection from the fistula tract that doesn't resolve on its own is the main reason surgery becomes necessary.

Signs You May Need Fistula Surgery

Symptoms include persistent pus or blood-stained discharge near the anus, recurring pain and swelling, skin irritation from constant drainage, and fever if infection flares up. A history of a previous anal abscess that keeps recurring or never fully heals is a strong indicator of an underlying fistula needing surgical evaluation.

Types of Fistula Surgery

Fistulotomy, the most common technique for simple fistulas, opens up the entire tract to let it heal from the inside out. For more complex fistulas involving significant sphincter muscle, surgeons may use a seton (a thread left in place to slowly cut through tissue or drain the tract) or newer sphincter-sparing techniques like LIFT (ligation of intersphincteric fistula tract) or fibrin glue to preserve continence.

Frequently Asked Questions

Will fistula surgery affect bowel control?

Simple fistulotomy for straightforward fistulas carries low risk to continence, but complex fistulas involving more sphincter muscle need careful technique selection to minimize this risk — your colorectal surgeon will choose the safest approach for your specific fistula.

How long does it take for a fistula to heal after surgery?

Healing time varies with fistula complexity, ranging from two to six weeks, and wound care with regular sitz baths is usually needed during recovery.

Can an anal fistula heal without surgery?

Fistulas rarely heal completely on their own since the tract keeps the area from closing properly, so surgery is generally required for a lasting cure, though the specific technique depends on the fistula's location and complexity.

Is fistula surgery painful?

The procedure is done under anesthesia so you won't feel pain during surgery, and post-operative discomfort is usually manageable with pain medication and warm sitz baths.

How successful is fistula surgery?

Success rates are good, around 85%, though complex or recurrent fistulas can sometimes need more than one procedure to achieve complete healing; a colorectal surgeon can assess your specific case.
