An apicoectomy is needed when a previous root canal treatment fails to resolve infection at the tip of the tooth root, often because of complex canal anatomy, a missed canal, or a fracture that a standard root canal cannot reach. Persistent infection, cysts, or granulomas at the root tip that don't respond to retreatment also make this surgery necessary. It is typically considered a last resort to save a tooth before extraction.

Signs You May Need Apicoectomy

Warning signs include recurring pain or tenderness around a previously root-canal-treated tooth, swelling or a pimple-like bump on the gum near the root, and sensitivity to biting or pressure. Persistent bad taste or drainage from a sinus tract, and X-ray evidence of a dark area at the root tip, also point to the need for this procedure.

Types of Apicoectomy

The classic approach removes the root tip along with any infected tissue and seals the canal from the root end with a retrograde filling. Modern endodontic microsurgery uses an operating microscope and ultrasonic instruments for greater precision and higher success, replacing the older traditional loupe-based technique. The choice depends on the tooth's location, root anatomy, and the extent of infection.

Frequently Asked Questions

Is apicoectomy painful?

The procedure is done under local anesthesia so you won't feel pain during it, and post-operative discomfort is usually mild and controlled with standard pain medication.

How successful is an apicoectomy?

Success rates are around 85%, especially with microsurgical techniques, but your endodontist can assess your specific tooth's likelihood of success beforehand.

What is the alternative to apicoectomy?

The main alternative is tooth extraction followed by an implant or bridge; discuss both options with your dentist to see which preserves function better in your case.

How long is recovery?

Most people recover within 3-7 days with mild swelling, a soft diet, and ice packs, returning to normal activities quickly.

Will I need a crown after apicoectomy?

Not always — the tooth's existing restoration usually stays in place, but your dentist will check whether any additional restorative work is needed.
