Why Exploratory Laparotomy Is Needed

Exploratory laparotomy is performed as an emergency when a patient has significant abdominal trauma, unexplained internal bleeding, or signs of an acute abdomen such as a perforated organ or bowel obstruction that cannot be safely diagnosed or managed with imaging alone. It allows surgeons to directly inspect and treat life-threatening abdominal injuries or infections.

Signs You May Need Exploratory Laparotomy

Patients needing this surgery typically present with severe, worsening abdominal pain, a rigid or distended abdomen, signs of shock such as low blood pressure and rapid heart rate, or visible trauma from an accident or injury. Fever with peritonitis-like symptoms following a suspected organ perforation is another common trigger for emergency exploration.

Types of Exploratory Laparotomy

The approach can be a midline incision, giving the widest access for trauma and unclear diagnoses, or a targeted incision based on where the problem is suspected to be, such as the right lower quadrant for suspected appendicitis. In some centres, a diagnostic laparoscopy is attempted first to assess the extent of injury, converting to open laparotomy if the findings require more extensive repair.

Frequently Asked Questions

Is exploratory laparotomy always an emergency?

It is most often performed urgently in trauma or acute abdomen situations, though it can occasionally be planned when other diagnostic tests have failed to identify the cause of a serious abdominal problem.

How long is the recovery after this surgery?

Recovery typically takes 4 to 8 weeks depending on what was found and treated during surgery, with hospital stay length varying based on the severity of the underlying condition.

What are the main risks involved?

As with any major abdominal surgery, risks include infection, bleeding, and a longer recovery, but because this procedure is usually life-saving, the benefits typically far outweigh these risks in an emergency setting.

Will I have a large visible scar?

Because the incision needs to allow full access to abdominal organs, the scar is usually more noticeable than with keyhole surgery, though it fades considerably over time.

When can normal activities resume after this surgery?

Light activities can often resume within a few weeks, but heavy lifting and strenuous exercise should be avoided until your surgeon confirms the abdominal wall has fully healed, usually around 6 to 8 weeks.
