What is Lumbar Disc Prolapse?
Lumbar disc prolapse, commonly known as a slipped or herniated disc, occurs when the soft, gel-like center of an intervertebral disc in the lower back pushes through a tear in its tougher outer layer, often pressing on nearby spinal nerves. This can cause localized back pain as well as radiating pain, numbness, or weakness along the path of the affected nerve, most classically down the leg in a pattern known as sciatica. It commonly results from age-related disc degeneration combined with mechanical stress such as heavy lifting or twisting. Most cases improve with conservative management, though severe or persistent cases may require surgical intervention.

Causes of Lumbar Disc Prolapse
Caused by age-related degeneration of the intervertebral disc, which loses water content and elasticity over time, making it more prone to tearing under mechanical stress such as heavy lifting, twisting motions, or repetitive strain. Risk factors include obesity, sedentary lifestyle, smoking, occupations involving heavy physical labor, and genetic predisposition to disc degeneration. Sudden trauma can also precipitate a prolapse in an already weakened disc.
Symptoms of Lumbar Disc Prolapse
Lower back pain, pain radiating down one leg (sciatica), numbness or tingling in the leg or foot, muscle weakness in the affected leg, pain that worsens with sitting, bending, or coughing, reduced range of motion in the lower back, in severe cases, loss of bladder or bowel control requiring emergency care
Treatment & Types of Lumbar Disc Prolapse in Telangana
Most cases are managed conservatively with rest, physical therapy, pain relief using nonsteroidal anti-inflammatory drugs or muscle relaxants, and gradual return to activity. Epidural steroid injections may be used for persistent nerve-related pain, while surgery such as microdiscectomy is reserved for cases with significant nerve compression, progressive weakness, or failure of conservative treatment. Most patients improve within six weeks with non-surgical care.
Lumbar disc prolapse is graded by the extent of disc material displacement: a bulge, a mild generalized outward extension, protrusion, localized displacement with the outer layer still intact, extrusion, where disc material breaks through the outer layer but remains connected, and sequestration, where a fragment fully separates from the disc, with more advanced grades more likely to cause significant nerve compression and require surgery.

Frequently Asked Questions
Do all slipped discs require surgery?
No, the large majority of cases, roughly 90%, improve within six weeks with conservative treatment like rest, physical therapy, and pain medication; surgery is reserved for significant nerve compression, progressive weakness, or failure of conservative treatment.
What does sciatica actually mean?
Sciatica describes pain that radiates down the leg along the path of the sciatic nerve, typically caused by a disc prolapse pressing on a nerve root in the lower back, and is the classic symptom pattern of this condition.
When is a slipped disc a medical emergency?
Loss of bladder or bowel control, or numbness in the groin area, can indicate a rare but serious complication called cauda equina syndrome and requires emergency surgical evaluation right away.
Can a disc prolapse heal on its own?
Yes, the body can often partially reabsorb the herniated disc material over weeks to months, which is why many cases improve significantly with conservative management alone rather than surgery.
What activities should I avoid with a slipped disc?
Heavy lifting, twisting motions, and prolonged sitting tend to worsen symptoms, so activity modification alongside guided physical therapy exercises is an important part of recovery.
