What is Perianal Abscess?

A perianal abscess is a painful collection of pus that forms near the anus, typically resulting from infection of the small glands inside the anal canal that become blocked and infected. It causes significant localized pain, swelling, and often fever, and requires prompt drainage to prevent the infection from spreading further into surrounding tissue. Left untreated, a substantial proportion of perianal abscesses progress to form a chronic anal fistula, an abnormal tunnel connecting the infected gland to the skin surface. It is more common in men and in individuals with certain underlying bowel conditions.

Severity

Moderate

Prevalence

Moderately common, more frequent in men, particularly those between 20 and 60 years of age

Contagious

Causes of Perianal Abscess

Most commonly caused by infection of the anal glands lining the anal canal, often due to blocked ducts that allow bacterial overgrowth. Risk factors include inflammatory bowel disease such as Crohn's disease, diabetes, immunosuppression, recent anal trauma or surgery, and receptive anal intercourse. Poor hygiene and chronic constipation with straining can also contribute.

Symptoms of Perianal Abscess

Severe throbbing pain near the anus, swelling and redness around the anal area, fever and chills, difficulty or pain with sitting, pain during bowel movements, a palpable tender lump near the anus, discharge of pus if the abscess spontaneously drains

Treatment & Types of Perianal Abscess in Jaipur, Rajasthan

Prompt surgical incision and drainage under local or general anesthesia is the primary treatment and provides rapid symptom relief. Antibiotics are used as an adjunct, particularly in patients who are immunosuppressed, diabetic, or have spreading infection, but do not replace the need for drainage. Follow-up is important to monitor for and address the frequent development of an anal fistula.

Perianal abscesses are classified by their location relative to the anal sphincter muscles, such as perianal, the most common type and closest to the skin surface, ischiorectal, intersphincteric, and supralevator abscesses, with deeper types being less common but often requiring more complex surgical management.

Frequently Asked Questions

Will antibiotics alone cure a perianal abscess?

No, antibiotics are used as an adjunct but do not replace the need for surgical incision and drainage, which is the primary and most effective treatment for providing rapid symptom relief.

Why do perianal abscesses often lead to a fistula?

A substantial proportion of drained abscesses leave behind a small tunnel connecting the infected gland to the skin surface, called an anal fistula, which is why follow-up after drainage is important to catch and address this common complication.

Who is at higher risk of developing a perianal abscess?

Men, and people with inflammatory bowel disease like Crohn's disease, diabetes, or a weakened immune system, are at higher risk, along with those with recent anal trauma or surgery.

Is perianal abscess surgery a major operation?

No, incision and drainage is typically a quick procedure done under local or general anesthesia and provides fast pain relief, though some cases with a resulting fistula may need a further procedure later.

How can I reduce my risk of a perianal abscess?

Maintaining good anal hygiene, promptly treating diarrhea and constipation, and managing underlying conditions like diabetes and inflammatory bowel disease can help reduce the risk.
