What is Cyclospora?

Cyclosporiasis is an intestinal infection caused by the parasite Cyclospora cayetanensis, typically contracted from consuming contaminated food or water. It causes prolonged diarrheal illness.

Severity

Moderate

Prevalence

Outbreaks linked to imported fresh produce in the US and Canada; endemic in tropical and subtropical regions

Contagious

Causes of Cyclospora

Caused by the parasite Cyclospora cayetanensis, usually through eating fresh produce (raspberries, basil, lettuce, snow peas) or drinking water contaminated with Cyclospora oocysts.

Symptoms of Cyclospora

Watery diarrhea (sometimes explosive), loss of appetite, weight loss, stomach cramps, bloating, nausea, vomiting, fatigue, low-grade fever, muscle aches. Symptoms may cycle (relapse and remission).

Treatment & Types of Cyclospora in Devla, Nashik, Maharashtra

Trimethoprim-sulfamethoxazole (TMP-SMX/Bactrim) for 7-10 days. Fluid replacement for dehydration. No proven alternative treatment for those who cannot take TMP-SMX.

Cyclosporiasis does not have distinct clinical subtypes but is described by its characteristic relapsing-remitting pattern, in which symptoms can improve and then recur in cycles over several weeks if untreated, distinguishing it from most other short-lived foodborne illnesses. Cases are also classified as sporadic or outbreak-associated, the latter typically traced to a specific batch of imported fresh produce.

Frequently Asked Questions

How do you get cyclosporiasis?

It is usually acquired by eating fresh produce such as berries, herbs, or leafy greens contaminated with Cyclospora oocysts, or drinking contaminated water, most often linked to imported produce in outbreak investigations.

How long does cyclosporiasis last if untreated?

Symptoms can persist for weeks and even relapse in cycles without specific treatment, which is why a course of trimethoprim-sulfamethoxazole is usually recommended rather than waiting for it to resolve alone.

Can cyclosporiasis spread from person to person?

Direct person-to-person spread is unlikely because the oocysts need time in the environment to become infectious, so most cases are linked to contaminated food or water rather than close contact.

What if I'm allergic to the usual antibiotic for cyclosporiasis?

There is no proven equally effective alternative to trimethoprim-sulfamethoxazole, so a doctor may consider desensitization or alternative supportive strategies on a case-by-case basis.

How can travelers avoid cyclosporiasis?

Washing fresh produce thoroughly, drinking treated or bottled water, and being cautious with raw produce in areas where the parasite is common can reduce risk while traveling.
