What is Gangrene?

Gangrene is the death of body tissue caused by a critical loss of blood supply, often affecting the extremities such as the toes, feet, or fingers, and sometimes internal organs. It can develop as a complication of severe peripheral arterial disease, diabetes, serious infection, or traumatic injury that disrupts blood flow to the affected tissue. Gangrene is classified as dry, where tissue slowly dies without infection, or wet and gas gangrene, which involve bacterial infection and can spread rapidly, becoming life-threatening within hours. It represents a surgical emergency requiring urgent treatment to prevent systemic infection and further tissue loss.

Severity

Severe

Prevalence

Uncommon but a serious complication seen predominantly in diabetics and those with severe peripheral arterial disease

Contagious

No

Causes of Gangrene

Most commonly results from severely compromised blood flow due to peripheral arterial disease, diabetes-related vascular and nerve damage, or a blood clot cutting off circulation. Serious bacterial infections, deep wounds, frostbite, and severe trauma that damages blood vessels can also cause gangrene by depriving tissue of oxygen. Risk factors include diabetes, smoking, obesity, immunosuppression, and untreated peripheral arterial disease.

Symptoms of Gangrene

Discoloration of the skin ranging from pale to blue, purple, black, or bronze, severe pain followed by numbness as tissue dies, a foul-smelling discharge in infected gangrene, swelling and blistering of the skin, coolness of the affected area, fever and chills if infection has spread, crackling sensation under the skin in gas gangrene

Treatment & Types of Gangrene in Maharashtra

Treatment requires urgent surgical removal of the dead tissue, known as debridement, to prevent the spread of infection, and in severe cases, amputation of the affected part may be necessary. Broad-spectrum antibiotics are used promptly for infected gangrene, and procedures to restore blood flow, such as angioplasty or bypass surgery, are performed when poor circulation is the underlying cause. Hyperbaric oxygen therapy may support healing in select cases.

Gangrene is classified as dry gangrene, where tissue dies slowly from poor blood flow without bacterial infection and progresses gradually, and wet or gas gangrene, involving bacterial infection that can spread rapidly and become life-threatening within hours, requiring emergency treatment. It is further described by whether it's a localized process affecting an extremity or has progressed to cause systemic infection (sepsis).

Frequently Asked Questions

Is all gangrene an emergency?

Wet and gas gangrene are surgical emergencies requiring immediate treatment due to rapid infection spread, while dry gangrene, though still serious, tends to progress more slowly, but any gangrene needs urgent medical evaluation.

Can gangrene be treated without amputation?

In some cases, especially when caught early or when blood flow can be restored through a vascular procedure, tissue can sometimes be saved, but extensive or infected tissue death often does require surgical removal of the affected area.

Why are diabetics at higher risk of gangrene?

Diabetes causes both nerve damage, which allows injuries to go unnoticed, and blood vessel damage, which impairs healing and circulation, together creating the conditions that allow gangrene to develop from minor untreated wounds.

What are the earliest warning signs of gangrene?

Skin discoloration, unusual coolness, severe pain followed by numbness, and any foul-smelling discharge from a wound are early warning signs that need immediate medical attention before the condition progresses.

Can gangrene be prevented in people with diabetes or poor circulation?

Yes, strict blood sugar control, daily foot inspection, prompt treatment of any wound, and quitting smoking meaningfully reduce the risk of gangrene developing in at-risk individuals.
