What is Scrub Typhus?

Scrub Typhus is an acute infectious disease caused by the bacterium Orientia tsutsugamushi, transmitted to humans through the bite of infected chigger mites found in areas with dense vegetation. The disease is characterized by high fever, a distinctive black scab called an eschar at the bite site, and can progress to severe multi-organ complications if untreated. It is endemic to rural and semi-rural parts of Asia, including many regions of India, particularly during and after the monsoon season. Early antibiotic treatment is highly effective and dramatically reduces the risk of severe complications.

Causes of Scrub Typhus

Caused by infection with Orientia tsutsugamushi bacteria, transmitted through the bite of infected larval mites, commonly called chiggers, which live in areas of dense scrub vegetation, grasslands, and forest edges. Agricultural workers, and those spending time in rural vegetation-dense areas during monsoon season, face the highest risk.

Symptoms of Scrub Typhus

High fever, headache, a dark scab (eschar) at the bite site, muscle aches, swollen lymph nodes, skin rash, cough, confusion in severe cases, abdominal pain

Treatment & Types of Scrub Typhus in Shivamogga, Karnataka

Treatment with doxycycline is highly effective and typically leads to rapid improvement when started early in the course of illness. Severe cases with organ complications require hospitalization and supportive care, including management of respiratory or kidney failure if present.

Scrub typhus is generally described by disease severity rather than distinct disease subtypes, ranging from mild illness with fever and the characteristic eschar that resolves with prompt antibiotic treatment, to severe disease involving complications such as acute respiratory distress, kidney failure, or meningoencephalitis when treatment is delayed. It can also be considered by exposure pattern, since cases are strongly seasonal, clustering during and after the monsoon when chigger mite activity in vegetation is highest.

Frequently Asked Questions

What is the black scab (eschar) doctors look for?

The eschar is a dark, painless scab that forms at the site of the chigger mite bite and is a highly characteristic and useful diagnostic clue, though it isn't always found or easily visible depending on its location on the body. Its presence alongside fever strongly points a doctor toward considering scrub typhus in an endemic area.

Is scrub typhus curable?

Yes, it is highly treatable, and doxycycline typically leads to rapid improvement when started early in the illness, making prompt recognition and treatment far more important than any long-term concern about the infection itself. Severe cases with organ complications need hospitalization but still generally respond well to treatment.

Can scrub typhus be prevented?

Wearing protective clothing covering the arms and legs, using insect repellent, and avoiding sitting or lying directly on grass in vegetation-dense areas during and after monsoon season are the main preventive measures, since there's no widely available vaccine. Clearing vegetation around homes in endemic areas also reduces exposure risk.

Why is scrub typhus more common after the monsoon?

Chigger mites that transmit the infecting bacteria thrive in dense vegetation and grasslands that flourish during and after monsoon rains, so agricultural workers and people spending time outdoors in rural areas face the highest seasonal risk. This seasonal pattern is a key reason doctors in endemic regions consider it during monsoon-related fever evaluations.

Can scrub typhus happen more than once?

Yes, immunity after infection is not fully protective against all strains, so repeat infections are possible, particularly with continued exposure to endemic vegetation-dense areas. Continuing preventive measures such as protective clothing and repellent use remains important even after a prior infection.
