What is Liver Cancer?
Liver cancer refers to a malignant tumour that originates in the liver, with hepatocellular carcinoma being the most common type and typically arising in a liver already damaged by chronic disease such as cirrhosis. The vast majority of liver cancer cases develop against a background of long-standing liver injury from chronic hepatitis B or C infection, alcohol-related liver disease, or fatty liver disease, all of which create an environment conducive to the abnormal cell changes that lead to cancer. Because early liver cancer often causes no symptoms, it is frequently diagnosed at a more advanced stage unless patients with known risk factors, such as cirrhosis, undergo regular surveillance imaging and blood testing. Treatment options and prognosis depend heavily on the size and number of tumours, whether the cancer has spread beyond the liver, and the underlying degree of liver function impairment. Prevention through hepatitis vaccination and management of chronic liver disease remains the most effective strategy against this cancer.

Uncommon in the general population but significantly elevated in those with cirrhosis or chronic viral hepatitis, a major risk group in India

Causes of Liver Cancer
Most commonly arises in the setting of chronic liver disease and cirrhosis caused by long-standing hepatitis B or C infection, heavy alcohol use, or non-alcoholic fatty liver disease, all of which cause repeated liver cell injury and regeneration that can lead to cancerous changes over time. Exposure to aflatoxins, toxins produced by mould that can contaminate improperly stored grains and nuts, is a recognised additional risk factor in some regions. Family history of liver cancer and certain inherited metabolic liver conditions can also increase risk.
Symptoms of Liver Cancer
Unintended weight loss, abdominal pain or swelling especially in the upper right area, loss of appetite, jaundice, fatigue, a palpable mass or lump in the abdomen, nausea, worsening of underlying liver disease symptoms
Treatment & Types of Liver Cancer in Karnataka
Treatment depends on tumour size, number, and the degree of underlying liver function, ranging from surgical removal or liver transplantation for early-stage disease in appropriate candidates, to localised treatments such as radiofrequency ablation or transarterial chemoembolisation for tumours not suitable for surgery. Targeted therapies and immunotherapy are used for more advanced disease that has spread beyond the liver. Regular surveillance with imaging and blood tests in high-risk patients, particularly those with cirrhosis, allows detection at earlier, more treatable stages.
The most common type of liver cancer is hepatocellular carcinoma, arising from the liver's main functional cells and typically developing in a liver already damaged by cirrhosis; a less common type, cholangiocarcinoma or bile duct cancer, arises from the cells lining the bile ducts within or near the liver. Liver cancer can also be primary, originating in the liver itself, or secondary and metastatic, where cancer from another organ has spread to the liver, which is actually more common than primary liver cancer overall.

Frequently Asked Questions
Why does liver cancer usually develop in people with cirrhosis?
Chronic liver injury and the constant cycle of cell damage and regeneration seen in cirrhosis creates an environment where cells are more prone to developing the mutations that lead to cancer, which is why people with cirrhosis undergo regular cancer surveillance.
Can hepatitis B vaccination really prevent liver cancer?
Yes, since chronic hepatitis B infection is a major cause of liver cancer, vaccination substantially reduces this risk long-term and is one of the most effective cancer-prevention vaccines available.
How is liver cancer monitored in high-risk patients?
Regular ultrasound and blood tests measuring a tumor marker called alpha-fetoprotein are used every six months in patients with cirrhosis or chronic hepatitis to catch tumors at an earlier, more treatable stage.
Is liver cancer curable?
Early-stage liver cancer confined to the liver can potentially be cured with surgical removal or liver transplantation in appropriate candidates, though more advanced disease is managed with treatments aimed at controlling rather than eliminating the cancer.
What lifestyle changes help protect the liver from cancer risk?
Limiting alcohol intake, maintaining a healthy weight to prevent fatty liver disease, avoiding mould-contaminated grains and nuts, and seeking treatment for chronic viral hepatitis are the most impactful preventive steps.
