What is Spinal Cord Compression?

Spinal cord compression occurs when pressure is applied to the spinal cord from a nearby structure, such as a herniated disc, bone spur, tumour, infection, or traumatic injury, disrupting the normal transmission of nerve signals between the brain and the rest of the body. It can develop gradually, as with degenerative spinal conditions, or suddenly, as with trauma or an expanding tumour, and the resulting neurological damage can become permanent if not treated promptly. The severity and pattern of symptoms depend on the exact level and degree of compression along the spinal cord. It is considered a neurosurgical emergency when it develops or worsens rapidly.

Severity

Severe

Prevalence

Uncommon overall but a recognized emergency complication of several common conditions including cancer and severe degenerative spine disease

Contagious

Causes of Spinal Cord Compression

Common causes include degenerative spine disease such as severe disc herniation or spinal stenosis, spinal tumours either primary or metastatic, traumatic injury from accidents or falls, and infections such as spinal abscess. Risk factors include osteoporosis, prior spinal surgery, cancer with a tendency to spread to bone, and age-related degenerative changes. The compression physically restricts blood flow and nerve conduction within the cord.

Symptoms of Spinal Cord Compression

Progressive weakness or numbness in the arms or legs, pain in the neck or back that may radiate, difficulty walking or loss of coordination, loss of bladder or bowel control, altered sensation such as tingling or burning, muscle spasms, in severe cases, paralysis

Treatment & Types of Spinal Cord Compression in Dera, Delhi

Emergency cases, particularly from trauma or rapidly progressive tumour compression, require urgent surgical decompression to relieve pressure on the spinal cord and prevent permanent damage, often combined with corticosteroids to reduce swelling. Chronic degenerative compression is managed with physical therapy, pain control, and surgery such as laminectomy or discectomy if symptoms are significant or progressive. Treatment of the underlying cause, such as radiation or chemotherapy for tumour-related compression, is essential alongside decompression.

Spinal cord compression is classified by underlying cause as degenerative, from conditions like severe disc herniation or spinal stenosis developing gradually over time; neoplastic, from a spinal tumor either arising in the spine or spreading there from cancer elsewhere; traumatic, from sudden injury such as a fall or accident; and infectious, from a spinal abscess. It is also described by onset speed as acute, developing rapidly and requiring emergency treatment to prevent permanent damage, versus chronic, developing slowly enough to allow more measured evaluation and treatment planning.

Frequently Asked Questions

Why is spinal cord compression sometimes an emergency and other times not?

Rapidly developing compression, such as from trauma or an expanding tumor, is a neurosurgical emergency because the resulting neurological damage can become permanent within hours if not treated promptly, while slowly progressing degenerative compression allows more time for evaluation and staged treatment. This is why sudden new weakness or loss of bladder control always warrants immediate emergency care.

Can spinal cord compression be reversed with treatment?

Many symptoms improve significantly with prompt decompression, especially when treatment happens before permanent nerve damage sets in, though some deficits may be permanent if compression was severe or present for a long time before treatment. This is why early evaluation of new or worsening neurological symptoms is strongly emphasized.

What are the warning signs that should prompt immediate emergency care?

New loss of bladder or bowel control, rapidly progressing weakness or numbness in the arms or legs, and difficulty walking or loss of coordination are red flags that suggest significant spinal cord compression requiring urgent evaluation. These symptoms should never be watched and waited on, given how quickly permanent damage can occur.

Is spinal cord compression common in cancer patients?

It's a recognized and important complication in patients with cancers that tend to spread to bone, since a spinal tumor can compress the cord as it grows, which is why cancer patients at risk are monitored closely for new back pain or neurological symptoms. Prompt evaluation of new back pain in someone with a cancer history is particularly important.

What treatments are used for spinal cord compression?

Emergency cases require urgent surgical decompression, often combined with corticosteroids to reduce swelling, while chronic degenerative compression is managed with physical therapy, pain control, and surgery such as laminectomy or discectomy if symptoms are significant. Treatment of the underlying cause, such as radiation or chemotherapy for a tumor, is also essential alongside decompression.
