What is Undescended Testis?

Undescended testis, medically known as cryptorchidism, is a condition in which one or both testicles fail to move down into the scrotum before birth, remaining instead in the abdomen or groin. It is one of the most common congenital conditions affecting male infants, particularly those born prematurely, and while many cases resolve spontaneously within the first few months of life as the testicle continues to descend naturally, those that do not descend by around six months typically require treatment. Leaving the condition uncorrected increases the long-term risk of reduced fertility and testicular cancer, making timely evaluation and treatment important.

Severity

Mild

Prevalence

Common — affects approximately 1-4% of full-term male infants and a higher proportion of premature male infants

Contagious

Causes of Undescended Testis

Results from incomplete descent of the testicle from its origin near the kidney down through the abdomen and into the scrotum during fetal development, a process normally completed by birth or shortly after. Risk factors include premature birth, low birth weight, a family history of undescended testis, and maternal exposure to certain hormones or environmental factors during pregnancy. The exact combination of hormonal and mechanical factors that leads to incomplete descent is not always identifiable in an individual case.

Symptoms of Undescended Testis

Absence of one or both testicles from the scrotum, an empty or underdeveloped appearance of the scrotum on the affected side, a palpable testicle in the groin that cannot be guided down into the scrotum, asymmetry between the two sides of the scrotum, typically no pain or discomfort associated with the condition itself

Treatment & Types of Undescended Testis in Deoli, Delhi

If the testicle has not descended on its own by around six months of age, surgical correction called orchidopexy is recommended, in which the testicle is carefully moved into the scrotum and secured in place. Early surgery, ideally by 12 to 18 months of age, improves long-term fertility potential and allows for easier future examination to monitor for testicular cancer risk. Hormonal therapy is occasionally used but is generally less effective than surgery and used only in select cases.

Undescended testis is classified by location as inguinal, where the testicle is palpable in the groin canal, the more common presentation, versus intra-abdominal, where the testicle remains higher up and is not palpable on examination, generally requiring more complex surgical approaches. It is also described as unilateral, affecting only one testicle, or bilateral, affecting both, with bilateral cases sometimes prompting additional hormonal evaluation to rule out other underlying conditions.

Frequently Asked Questions

Will an undescended testicle come down on its own?

Many cases resolve spontaneously within the first few months of life as the testicle continues to descend naturally, but those that do not descend by around six months typically require surgical treatment rather than continued waiting. This is why follow-up examination in early infancy is important to confirm whether spontaneous descent has occurred.

Why does timing of surgery matter so much?

Early surgery, ideally by 12 to 18 months of age, improves long-term fertility potential and allows for easier future examination to monitor for testicular cancer risk, both of which are compromised the longer the testicle remains undescended. This is why pediatric urologists generally recommend not delaying surgical correction beyond the recommended window.

Does having an undescended testicle increase cancer risk later in life?

Yes, leaving the condition uncorrected increases the long-term risk of both reduced fertility and testicular cancer, which is why timely evaluation and treatment is emphasized rather than simply monitoring indefinitely. Even after correction, men with a history of undescended testis are often advised to continue regular self-examination.

Is undescended testis hereditary?

A family history of undescended testis is a recognized risk factor, alongside premature birth, low birth weight, and maternal exposure to certain hormones or environmental factors during pregnancy, suggesting a combination of genetic and environmental influences. The exact combination of factors leading to incomplete descent isn't always identifiable in an individual case.

How effective is surgery for correcting this condition?

Surgical correction, called orchidopexy, in which the testicle is carefully moved into the scrotum and secured in place, is generally very effective, and outcomes are best when performed within the recommended early treatment window. Hormonal therapy is occasionally used but is generally less effective than surgery and reserved for select cases.
