The commonest causes are insufficient or poor-quality sleep, iron-deficiency anaemia, thyroid dysfunction, depression or anxiety, vitamin B12 or D deficiency, uncontrolled diabetes, and sleep apnoea. Persistent fatigue beyond 2–3 weeks warrants blood tests rather than more caffeine.

A reasonable first panel: complete blood count, ferritin, TSH, fasting glucose or HbA1c, vitamin B12 and vitamin D, plus creatinine and liver enzymes.

If bloods are normal, look at sleep quality and mood. Obstructive sleep apnoea and depression are the two commonest causes of fatigue with a clean blood report.
