Why Glaucoma Surgery (Trabeculectomy) Is Needed

Trabeculectomy is recommended for glaucoma patients whose intraocular pressure remains dangerously high despite maximum medical therapy with eye drops or after laser treatment has failed to provide adequate control. It is performed to prevent further, often irreversible, damage to the optic nerve and progressive vision loss.

Signs You May Need Glaucoma Surgery (Trabeculectomy)

Patients typically have a known diagnosis of glaucoma with progressively worsening visual field loss, or an eye specialist finds persistently elevated eye pressure on repeated checks despite drops. In advanced or acute cases, symptoms can include eye pain, redness, blurred vision, or halos around lights, particularly in acute angle-closure glaucoma.

Types of Glaucoma Surgery (Trabeculectomy)

Traditional trabeculectomy creates a new drainage channel under a partial-thickness scleral flap to let fluid drain and lower pressure, sometimes combined with an antimetabolite like mitomycin-C to prevent scarring of the new channel. Alternative or combined approaches include tube shunt (glaucoma drainage device) surgery for more complex cases, and minimally invasive glaucoma surgery (MIGS) techniques for milder disease.

Frequently Asked Questions

Will this surgery cure my glaucoma?

Trabeculectomy does not cure glaucoma but effectively lowers eye pressure to slow or halt further optic nerve damage; any vision already lost generally cannot be restored, which is why early treatment is important.

Is the surgery painful?

The procedure is done under local anaesthesia with sedation, so you won't feel pain during surgery, though some discomfort, redness, and light sensitivity are common during the healing period.

How long is the recovery period?

Most patients stay in hospital for 1 to 2 days, and full recovery with stable pressure control can take 6 to 8 weeks, during which frequent follow-up visits are essential.

Will I still need eye drops after surgery?

Some patients can reduce or stop their glaucoma drops after successful surgery, while others may still need medication for pressure control, so this depends on individual response and follow-up findings.

What are the main risks of trabeculectomy?

Risks include infection, bleeding, or the new drainage channel scarring over and losing effectiveness over time, which is why regular follow-up with your ophthalmologist is crucial after this surgery.
