Laparoscopic sleeve gastrectomy is now the standard approach, removing about 75-80% of the stomach to leave a narrow, banana-shaped "sleeve," which reduces food intake and also lowers levels of the hunger hormone ghrelin. In rare cases where laparoscopic access is difficult, an open approach may be used, though minimally invasive surgery is strongly preferred given its faster recovery and lower complication rate.
