An ileal conduit is created after the bladder has been surgically removed (usually for bladder cancer) or when the bladder no longer functions and needs to be bypassed, such as from severe neurological damage or radiation injury. It provides a way for urine to exit the body when the natural bladder can no longer store or empty urine safely.

Signs You May Need Ileal Conduit

Because this procedure follows bladder removal or bladder failure, the underlying warning signs are those of the condition necessitating cystectomy or bladder dysfunction — blood in urine, recurrent infections, incontinence, or inability to empty the bladder despite catheterization. Patients reach this point after the primary bladder condition has already been diagnosed and treated to the extent possible.

Types of Ileal Conduit

The standard technique uses a short segment of the ileum (small intestine) as a conduit, connecting the ureters (from the kidneys) to one end and bringing the other end out through a small opening (stoma) on the abdominal wall where urine collects in an external pouch. Alternative urinary diversions, such as a continent cutaneous reservoir or an orthotopic neobladder, may be considered instead in appropriately selected patients who want to avoid an external bag.

Frequently Asked Questions

Will I need to wear a bag permanently after an ileal conduit?

Yes, urine drains continuously into an external collection bag attached to the stoma, though modern appliances are discreet and a stoma nurse will teach you how to manage it comfortably in daily life.

How long does it take to adjust to life with an ileal conduit?

Most people adapt to stoma care within a few weeks with guidance from a stoma care nurse, and many return to normal work, travel, and social activities once comfortable with the routine.

What are the risks associated with an ileal conduit?

Risks include stoma-related skin irritation, urinary infections, and, over the longer term, potential issues like stone formation or narrowing at the connection sites, which are monitored through regular urology follow-up.

How long is recovery after ileal conduit surgery?

Hospital stay is typically seven to ten days, since it's usually performed alongside bladder removal, with full recovery and adaptation to the stoma taking six to eight weeks.

Can an ileal conduit later be converted to a neobladder?

In some cases, conversion may be technically possible, but it's a complex decision depending on individual anatomy and health, so this should be discussed thoroughly with a urologic surgeon experienced in urinary reconstruction.
