Why Coronary Bypass Surgery (CABG) Is Needed

Bypass surgery becomes necessary when coronary arteries are blocked in multiple locations or in complex patterns, such as at the left main artery, that make balloon angioplasty and stenting less suitable or durable. It is also preferred in patients with diabetes and widespread coronary disease, or after a large heart attack with ongoing risk of recurrent blockage.

Signs You May Need Coronary Bypass Surgery (CABG)

Persistent chest pain despite optimal medication, breathlessness with minimal exertion, and angiography showing extensive blockages across several coronary arteries typically lead a cardiologist to recommend this surgery. Patients who have already had a heart attack and remain at high risk of another due to unresolved blockages are also common candidates.

Types of Coronary Bypass Surgery (CABG)

The grafts used to bypass blocked arteries can come from the patient's own leg vein (saphenous vein graft), arm vein, or chest artery (internal mammary artery), with arterial grafts generally lasting longer than vein grafts. Surgery may be done using a heart-lung machine (on-pump) or on a beating heart (off-pump), depending on the surgeon's assessment and the complexity of the case.

Frequently Asked Questions

How many bypasses might I need?

This depends entirely on how many arteries are significantly blocked, ranging from a single bypass to three or four, as determined by your angiogram.

Is the recovery from open heart surgery difficult?

The first few weeks involve gradual activity increases and some discomfort at the incision site, but most patients see steady improvement, with full recovery generally by 6-12 weeks.

When can I drive again after bypass surgery?

Most surgeons advise waiting about 4 weeks before driving, once the breastbone has had time to begin healing.

Will my heart function improve after bypass surgery?

Most patients experience significant relief from angina and improved exercise tolerance once blood flow is restored, though your cardiologist will assess your specific heart function before and after.

What can I do to protect the new grafts long-term?

Controlling blood pressure, cholesterol, and blood sugar, not smoking, staying active, and taking prescribed medications consistently are key to keeping your grafts healthy for years to come.
