Why Endotracheal Intubation Is Needed
Endotracheal intubation is performed when a patient cannot protect or maintain their own airway, such as during cardiac or respiratory arrest, severe trauma, drug overdose, or profound unconsciousness. It is also required to deliver mechanical ventilation during major surgery under general anaesthesia or in critically ill patients with respiratory failure.
Signs You May Need Endotracheal Intubation
Clinical signs prompting intubation include a severely reduced level of consciousness with an inability to protect the airway, gasping or absent breathing, oxygen levels that remain low despite supplemental oxygen, and signs of impending airway obstruction such as severe swelling from an allergic reaction or burns. It is also planned electively before surgeries requiring general anaesthesia.
Types of Endotracheal Intubation
Orotracheal intubation, where the tube is passed through the mouth, is the most common emergency approach, while nasotracheal intubation goes through the nose and is used in select surgical or dental settings. Rapid Sequence Induction (RSI) is a specific technique combining fast-acting sedative and paralytic drugs to secure the airway quickly in emergencies, and awake fibre-optic intubation is used for patients with an anticipated difficult airway.

In emergencies and planned surgeries, patients are typically sedated or already unconscious, so they do not feel or remember the procedure; awake intubation is reserved for special cases with a difficult airway.
How long does a breathing tube usually stay in place?
Duration depends entirely on the underlying condition, ranging from a few hours during surgery to several days or longer in intensive care for patients with severe respiratory failure.
What are the risks of intubation?
Possible risks include minor throat soreness or hoarseness afterward, and less commonly, injury to teeth or vocal cords or complications from the sedative drugs, all of which the treating anaesthesiologist or intensivist monitors closely.
Will there be pain after the tube is removed?
A sore throat and mild hoarseness for a few days is common and usually resolves on its own; persistent voice changes should be reported to your doctor.
How is the patient cared for while intubated?
Intubated patients are managed in an ICU setting with continuous monitoring, sedation as needed, and a ventilator to support breathing until they can breathe safely on their own.
