This emergency surgery is performed when the spermatic cord twists, cutting off blood supply to the testicle — usually due to a congenital anatomical predisposition (bell-clapper deformity) that allows the testicle to rotate more freely than normal, sometimes triggered by physical activity, trauma, or occurring spontaneously during sleep.

Types of Testicular Torsion Surgery
The standard procedure is surgical detorsion (untwisting) followed by orchiopexy, which stitches the testicle to the inside of the scrotum to prevent future twisting; if the testicle is no longer viable due to prolonged loss of blood supply, orchiectomy (removal) may be necessary, and the unaffected testicle is often fixed in place preventively at the same time.
