If the testicle has not descended on its own by around six months of age, surgical correction called orchidopexy is recommended, in which the testicle is carefully moved into the scrotum and secured in place. Early surgery, ideally by 12 to 18 months of age, improves long-term fertility potential and allows for easier future examination to monitor for testicular cancer risk. Hormonal therapy is occasionally used but is generally less effective than surgery and used only in select cases.

Undescended testis is classified by location as inguinal, where the testicle is palpable in the groin canal, the more common presentation, versus intra-abdominal, where the testicle remains higher up and is not palpable on examination, generally requiring more complex surgical approaches. It is also described as unilateral, affecting only one testicle, or bilateral, affecting both, with bilateral cases sometimes prompting additional hormonal evaluation to rule out other underlying conditions.
