Excess sebum, blocked follicles from abnormal keratin shedding, *Cutibacterium acnes* bacteria, and inflammation — driven by hormones and genetics. Diet plays a modest role (high glycaemic load and possibly skim milk). It is not caused by poor hygiene, and over-washing worsens it.
