Total colectomy is performed for severe ulcerative colitis that isn't controlled by medication, familial adenomatous polyposis (a hereditary condition with hundreds of precancerous polyps), extensive colorectal cancer, or Crohn's disease affecting the entire colon, all of which require removing the whole colon to control disease or prevent cancer.

Signs You May Need Total Colectomy

Warning signs include chronic bloody diarrhea, severe abdominal pain and cramping, significant weight loss, anemia from ongoing blood loss, and in emergencies, signs of toxic megacolon such as high fever and a rigid, distended abdomen.

Types of Total Colectomy

Options include total colectomy with ileostomy, where waste exits through an opening in the abdomen into a bag, and total colectomy with ileal pouch-anal anastomosis (a J-pouch), which creates an internal reservoir from small intestine allowing more normal bowel movements without a permanent external bag.

Frequently Asked Questions

Will I need a permanent colostomy bag?

Not necessarily — many patients, especially those with ulcerative colitis, are candidates for a J-pouch procedure that avoids a permanent external bag, though this depends on individual anatomy and disease.

How does life change after total colectomy?

Bowel habits change significantly, often with more frequent, looser stools, but most patients adapt well over several months and see major improvement in their underlying disease symptoms.

Is this surgery curative for ulcerative colitis?

Yes, removing the entire colon effectively cures ulcerative colitis since the disease is limited to the colon, unlike Crohn's disease which can recur elsewhere in the digestive tract.

How long is the recovery?

Recovery typically takes 6-8 weeks, with a hospital stay of about 7-10 days, and further recovery time if a second-stage pouch surgery is planned.

What are the main risks?

Risks include infection, pouch-related complications, and nutritional changes, all of which are managed closely by a colorectal surgery team afterward.
