Many cases, especially viral ones, resolve on their own with pain relief using appropriate analgesics and observation over 48 to 72 hours. Bacterial infections that persist or are severe are treated with a course of oral antibiotics. Recurrent or persistent fluid-filled infections that do not respond to medical treatment may require insertion of tympanostomy tubes through a minor surgical procedure to ventilate the middle ear and restore hearing.
Otitis media is classified as acute otitis media, a short-term infection with sudden onset of ear pain and fever, otitis media with effusion, involving persistent fluid behind the eardrum without active infection, often following an acute episode, and chronic suppurative otitis media, involving persistent drainage through a perforated eardrum, each requiring somewhat different management.
