PCOS, thyroid disorders, high prolactin, significant weight change, excessive exercise, stress, perimenopause, contraception, and structural causes like fibroids or polyps. Pregnancy should always be excluded first.

A reasonable workup: repeat pregnancy test, TSH, prolactin, FSH, LH and testosterone, with a pelvic ultrasound.
